Founded in 1940 by Gladys Bownes, Barnwell’s staff strives to continue Gladys’ vision of excellent nursing care coupled with the notion that a stay in a nursing home, for either long term care or short term rehabilitation, should be an experience filled with caring and respect for the dignity of the individual.

Shore View Nursing and Rehabilitation Center is proud to provide outstanding progressive short-term rehabilitation and compassionate long-term care.  Registered Nurses, Licensed Practical Nurses and Certified Nurse’s Aides are scheduled 24 hours a day, providing superior skilled nursing care around the clock for all of our patients.

Our talented and outstanding skilled nursing staff coordinates the individualized care plans for each patient and focuses on enabling individuals to acquire or relearn skills they need to return home as quickly and independently as possible. At Shore View, we are committed to promoting wellness and optimum quality of life for all patients under our care, in both our short-term rehabilitation and long-term care programs. The healthcare team at Shore View strives to restore each individual to an optimum level of function, while stressing independence and adaptability throughout his or her stay. 

SOCIAL SERVICES

We recognize that caring for a loved with health issues can be very stressful for concerned family members. Family caretakers are frequently faced with many insurance obstacles while making complex rehabilitative care decisions for their loved ones and are often forced with the difficult decision to transfer permanent care of a loved one to a nursing facility when they are no longer able to provide the needed care. The decision-making process is daunting without the full support of experienced professionals who can offer expertise and guidance to residents and family members.

At Barnwell Nursing and Rehabilitation Center, we understand that the transition from the hospital to a rehabilitation /nursing facility can be challenging.  Our Social Work department is comprised of professionals who bring a unique blend of experience, dedication and compassion to their residents and family members. An experienced and compassionate social worker will be made available to each resident and their family during this adjustment period to offer guidance and support during the transition process.

For short-term rehabilitation residents, a social worker will coordinate discharge planning shortly after admission to Barnwell.  Rehabilitation programs at Barnwell are focused on maximizing personal independence and returning residents home as quickly and safely as possible.  Social Workers provide each resident with a comprehensive discharge plan and assist in arranging support services that may be required at home. 

Our primary goal in discharge planning is to ensure that each resident returns to a safe and supportive home environment with the recommended support team in place. Once a resident has attained the goals established by the Rehabilitation Department and receives medical clearance from the Attending Physician to be discharged from Barnwell, the assigned social worker will set a discharge date with the resident and a designated representative.

As a resident at Barnwell, your social worker is an advocate and a liaison for you and your family and will work closely with you to ensure your needs are met and help you adjust seamlessly to your new home environment.

Family Support Groups are held monthly to offer additional assistance and useful resources.
